Description:
We have an exciting opportunity for a highly motivated Band 7 Registered Mental Health Nurse or Mental Health Allied Health Professional to join our All Age Continuing Care and Specialist Placements Team (AACC), based at the Compass Centre in Chatham, Kent.
You will be employed by NHS Kent and Medway Integrated Care Board (KMICB) and work primarily with patients originating from North Kent (Dartford, Gravesham and Swanley, Swale and Medway), placed in various geographical areas in England, but predominantly in Kent.
The All Age Continuing Care and Specialist Placements team function is to provide the total service delivery for all placements/care commissioned by the NHS Kent & Medway Integrated Care Board (KMICB) according to National Framework for NHS Continuing Healthcare and Funded Nursing Care (CHC/FNC). In addition, the team provides complex clinical case management for those eligible for NHS CHC following assessments and undertake quality reviews of all CHC and FNC eligible patients.
The team supports delivery of Personal Health Budgets (PHBs) and commissioning services and support for individuals who are eligible for funding under the National Framework for NHS Continuing Healthcare and NHS Funded Nursing Care. The team provides a variety of challenging and rewarding opportunities within a supportive and innovative environment.
The post holder will support the Associate Director and Assistant Head of AACC to manage Band 6 nurses and ensure correct processes are followed in the delivery of the National Framework for NHS Continuing Healthcare & NHS Funded Nursing Care.
- The post holder will be responsible for undertaking holistic assessments according to National Framework for NHS Continuing Healthcare and Funded Nursing Care and must have an understand of Section 117 Aftercare and how this interrelates with the CHC and FNC National Framework.
- The post holder will manage complex clinical cases, quality reviews, developing personalized care and support plans in the delivery of Personal Health Budgets (PHBs).
- Assist the Associate Director and Assistant Head of AACC in responding to enquires or requests for information from political and legal sources.
- Assist with the management of CHC appeals process by preparing cases and clinical information for appeals at internal and external panels.
- Keep accurate review documentation and robust electronic records at all times.

The hours are 37.5 per week and Monday to Friday 09.00 – 17.00 hours, though we can offer hybrid working and flexible options.
