BAND 6 COMMUNITY MENTAL HEALTH PRACTITIONER - WAVENEY ADULT COMMUNITY MENTAL HEALTH TEAM

Waveney Mental Health Team are looking for new Band 6 Community Mental Health Practitioners. You will be a qualified professional Nurse,
Occupational Therapist or Social Worker with experience of working in the community supporting working age adults with mental health problems
following the Care Programme Approach. The successful candidate will demonstrate a sound understanding of their role as a Care Coordinator
and join our friendly multidisciplinary team of Social Workers, O.T.s and Nurses who are experienced community practitioners. you also will be
part of the triage and assessment process within the team for referrals from the GP surgeries and other services. So, you will need to demonstrate
robust and sound assessment skills. Your potential team colleagues are Assistant Practitioners, Peer workers, Psychologists, Psychological
therapists, Carer’s support, Physical health lead, Employment worker, Psychiatrists and Therapists all ably supported by our admin staff.

Responsibilities:

MAIN DUTIES OF THE JOB

  • To undertake the planning and delivery of direct patient care with minimal supervision, ensuring continuity of practice and working within a multidisciplinary and multi-agency environment.
  • To manage a caseload and the provision of mental health care services to people with mental health care need.
  • To be responsible for accurate mental health clustering of all patients on admission, discharge and at review.
  • To ensure all mental health clustering information is passed onto other Trust teams or services on transfer of patients.
  • To ensure accuracy and quality of mental health clustering activity within the team.
  • Applicants must hold a full UK driving licence.
