Description:
JOB SUMMARY
We are looking for an enthusiastic and motivated Physiotherapist to join our Community Neuro Rehabilitation Team in Southampton.
CNRT is a friendly team that provides specialist community therapy intervention to patients with a wide variety of neurological conditions and presentations in the Southampton city and surrounding Hampshire region. You will work closely with the other Physiotherapists within the team and other multidisciplinary team members including Rehabilitation Consultants, Specialist Nurses, Occupational Therapists, Speech and Language Therapists, Psychologists, Rehabilitation Assistants, and Administrative support.
As well as being supported by the Team Leader, Senior Clinicians and Service Leads you will receive professional specific and CPD support from band 7/8 Physiotherapists within the service. The post holder will have post graduate experience which should include some specific neurological clinical experience and evidence of working within a multidisciplinary team. Experience of service improvement projects or audits is also desirable.
This is a community post and therefore access to a car for community visits is essential.

Responsibilities:
To work in our multi-professional Community Neuro Rehabilitation Team (CNRT)
Work in close collaboration with other relevant services/agencies e.g. acute rehab units, community rehab teams, virtual wards, rapid response etc.
Manage caseload of clients whilst working within the community and oversee caseload for rehabilitation assistants.
Undertake assessment and treatment of clients, who have complex / and or chronic presentation requiring specialist skills and determine profession specific and multidisciplinary rehabilitation interventions.
Plan and implement comprehensive programmes of intervention for clients who require intensive rehabilitation in the community setting.
Work with clients to develop greater self-management of health.
To manage and organise the teams caseload when the team leader is absent e.g. attending the MDT meeting, scheduling work for self and other team members, providing specialist advise to team members and external agencies
Carry out profession specific and general multidisciplinary rehabilitation procedures, many of which will require specialist skills and knowledge
Supervise junior members of staff, rehabilitation assistants and students on clinical placement.
Participate in and maintain own clinical professional development (CPD) by keeping abreast of any new developments e.g., active involvement in professional special interest groups and taking responsibility to incorporate into practice.
To actively contribute to team developments on a routine basis.
