Would your patients say that you’ve planned their safe and timely discharge? Have you been frustrated by the challenges faced in discharging your patients and want the opportunity to do something about that?
We are recruiting a Senior Onward Care Team clinician to join our team. This is an exciting time to join us as our One Gloucestershire ICS implements new ways of working that help us to embed #HomeFirst as our default discharge plan. We require dynamic, driven, positive people who embrace change and indeed, identify the change needed, to help us support even more of our patients to return home when they are ready to leave hospital.

To act as a resource providing advice and information to clinical teams to help identify patients with onward care needs and support the MDT to address those needs in a timely and responsive way in order to meet expected date of discharge as soon as they are medically optimised.
To work in an allocated clinical area, having oversight of the team’s workload within that area, pro-active support of less experienced or more junior members of staff in timely discharge planning, using a range of tools to help unblock barriers to discharge and when needed, escalate accordingly within appropriate channels.
Relevant experience working within patient flow, discharge planning or equivalent role in an NHS setting.
Awareness of the Governments Hospital Discharge Service Guidance and how this affects discharge planning.
Able to engage and motivate a range of staff to embrace different ways of working and thinking to improve onward care planning for patients.
We are a team of Nurses, Therapists and non-registered clinical staff who work across Gloucestershire Royal and Cheltenham General Hospitals who do a wonderful job of looking after each other.
We are constantly adapting to the changing environment of onward care planning from the Acute Hospital and pride ourselves on being at the forefront of cross-system conversations that influence service delivery.
We will need you to be dynamic, engaged, happy to question things in a way that brings people with you, flexible in your thinking and organised in your daily tasks. We will support you with your continued professional development, giving you the opportunity to look at specialist areas within discharge planning, expanding your skills, opening doors to help you to make changes for your patients.
