Description:

KEY RESULT AREAS The ability to lead and deliver safe care within a challenging and changing clinical environment, using equipment and resources safely and as appropriate. Advise the multi- disciplinary team on the HDS guidance, subsequent onward care options and how to access them. Undertake patient assessment using clinical skills to inform and define discharge and transfer pathways for patients in the allocated clinical area. To be involved with the creation, evaluation and implementation of cross system service improvement plans that specifically address onward care, including attendance at appropriate committees/forums to develop and maintain services.
To communicate effectively especially where patient ability to do so is compromised. To deliver and receive information which may cause emotional distress to all parties and be able to disseminate contentious and delicate information appropriately. To adhere to all Trust policies and procedures and professional code of conduct. To support Line Manager in recruitment, retention and performance related issues, including being regularly responsible for the supervision, education and mentorship for unqualified/qualified staff and students.
In accordance with the data protection act assist Line Manager in ensuring own and staff ability to input and retrieve patient information from computer based system, and maintain accurate and contemporaneous patient records. Ensure accurate information is maintained at all times for patient caseload. To provide clinical leadership that inspires and motivates others. Develop clinical competencies as appropriate to meet the needs of the service Work effectively as a liaison with other organisations eg.
Adult Social Care, Gloucestershire Integrated Brokerage, Gloucestershire Health and Care NHS Foundation Trust, Voluntary and Independent sectors and other community services Knowledge and understanding of safeguarding and how to apply relevant policy and procedure in practice Maintain relevant data, statistics, workload and workforce information for yourself and your team to support monitoring and performance management of the team. Identify trends within the clinical area to support business planning and to support the development of unscheduled model of care for Gloucestershire Respond to agreed escalation procedures during times of extreme capacity demands in the health and care system in Gloucestershire Ensure all health and safety legislation and departmental policies are implemented to promote a safe working environment. Take responsibility for own CPD by utilising competencies for career development, taking advantage of training opportunities as identified in your personal development plan, development of a professional portfolio
