Join our Connecting Families Team as a Senior Practitioner Social Worker!
Are you a dedicated and dependable Senior Practitioner Social Worker? We are looking for someone like you to support the Early Help Allocation Panel (EHAP) and make a real difference in the lives of children, young people, and their families. This post is 15 hours per week to be worked two days out of the following three - Monday, Tuesday or Friday.
KEY RESPONSIBILITIES:
- Prepare for and attend EHAP meetings
- Contact families to establish their needs and identify any issues or concerns
- Conduct home visits and arrange Lead Team Around the Family (TAF) meetings
- Complete step-down consultations with colleagues
- Produce detailed written reports and family assessments
PLEASE NOTE THAT THIS ROLE IS SUBJECT TO AN ENHANCED DBS CHECK.
