HOW YOU’LL MAKE A DIFFERENCE
As a Senior Practitioner in the Hospital / Community Transfer of Care Team you will make a difference by improving the quality of services provided to adults leaving hospital with care and support needs. You will make a difference by having a pivotal role in ensuring performance and quality standards across the Adult Social Care team are met and delivered to vulnerable adults in our community.
WHAT WE NEED FROM YOU
- You will hold a relevant professional qualification e.g. DipSW, CQSW, CSS , OT.Diploma and /or degree and be registered with the Health and Care Professions Council (HCPC).
- You will have relevant post qualifying experience in a social care setting and experience of dealing with complex cases.
- You will have comprehensive understanding of the Discharge to Assess Model and experience working in Hospital Discharge.
- You’ll be able to provide evidence of on-going continuous professional development, knowledge of current legislation and practice relating to social care with the ability to be able to assess and manage risk.
Please note that due to the nature of this role it is a requirement of employment that an enhanced Disclosure and Barring Service (DBS) check is obtained for this post.
You will be expected to travel throughout the authority and surrounding area so must have a full valid driving licence with regular access to a vehicle or have alternative appropriate means of travel.
Responsibilities:
The Hospital Social Care Team consists of Social Work Practitioners, Occupational Therapists, and Social Care Practitioners. We use the 3 Conversation model to work with people following discharge from hospital, completing conversations and assessments in multiple settings including care homes and rehabilitation units across the whole geographical area. We work closely with acute and community health partners, as a supportive and friendly team who are used to working at pace.
- Under the supervision of a Team Manager, you will take a proactive role in day to day management of the team including providing advice and support and supervision to team members
- You will liaise with Social Work or Occupational Therapy staff, and other associated staff in relation to social care assessments, support plans, practice and decision making.
- You will support staff with managing complex cases taking account of levels of risk, safeguarding and issues of mental capacity and DOLS as required.
- You will ensure effective prioritisation and allocation of work.
- You will ensure good quality assessments, care planning and intervention are completed meeting statutory requirements with the 3 conversation model.
- You will ensure high standards of data recording are maintained.
- On occasions you may be required to deputise for the Team Manager and attend meetings with other agencies.
- You will maintain and promote effective partnerships with relevant agencies including the ICB, Sirona and Acute Health Agencies to achieve best outcomes for service users. This may involve assisting in developing and implementing multi-agency policies and procedures.
- As an experienced practitioner you will keep your professional skills and knowledge of legislation and practice up to date and on occasions, you may be required to act as practice educator to qualifying and newly qualified staff.
