Description:

DESCRIPTION

The Senior Program Manager has oversight for all aspects of the Southern California Skilled Nursing Facility (SNF) Program in order to enable long-term and short-term goals to be achieved in a manner that is consistent with the Southern California Region. This position will work closely with Executive leadership in developing and maintaining the strategic direction of SNF Services for Heritage patients, physicians and care managers. They will collaborate with the Director of SNF Collaboratives and participate in the Southern California SNF Collaboratives to ensure good relationships with our partner SNFs and Heritage patients. The Senior Manager is accountable for achieving established performance targets through actively engaging multiple disciplines and professional staff with both internal and external stakeholders.

REQUIRED QUALIFICATIONS:

  • Bachelor’s Degree: Behavioral health-related field.
  • Upon request: Driving may be necessary as part of this role. Caregivers are required to comply with all state laws and requirements for driving. Caregivers will be expected to provide proof of driver license and auto insurance upon request. See policy for additional information
  • 5 years experience in a progressively responsible position requiring contact with various departments required.
  • 3 years Demonstrated experience leading complex improvement projects.
  • 3 years Demonstrated experience presenting to a variety of audiences.

PREFERRED QUALIFICATIONS:

  • Master’s Degree: related field
  • 5 years Demonstrated leadership experience including project management, change management, or quality management.
  • IAHSS Workplace Violence Prevention Certification
