Description:
To work as part of the multi-disciplinary team, providing a high standard of specialist palliative care to patients and their families. The Post Holder, in conjunction with the Ward Sister, is responsible for leading the team of Registered Nurses in the assessment, development, implementation and evaluation of holistic patient care. Clinical 1. To be responsible for working alongside the ward sister to lead the ward team whilst building successful links between key professionals to support and promote effective multi-disciplinary team working.
2. Communicate effectively and efficiently, whilst liaising with other members of the multi-disciplinary team to ensure patients receive seamless care within the hospice and other healthcare settings. 3. Responsible for maximising the use of clinical resources, implementing best practice in patient flow, admission and discharge arrangements.
4. To ensure the ward have adequate resources/equipment to provide a safe and effective service. 5. Responsible for ensuring the escalation of issues when a change to circumstances may affect safe practice 6.
Work with the ward MDT and Advanced nurse practitioner to ensure that patients are admitted appropriately and their needs are able to be met by the hospice facilities and care. 7. Participate in multi professional research and be responsible for ward based audits: with the ward sister develop and implement any action plans. 8.
To be Responsible for leading and supporting the ward team alongside the ward sister through the process of change, demonstrating tenacity, drive, professional integrity, balance and perspective. 9. Responsibility for identifying any Health and Safety risks and incidents within the clinical area; liaising with the Health and Safety manager as and when required. Escalating any maintenance issues in a timely way.
10. Ensure that all CQC reporting requirements are adhered to and the team are engaged with the key lines of enquiry. 11. Alongside the ward sister help motivate, develop, support and identify training needs for ward staff, through regular appraisals and meetings.
12. Support the ward sister with implementing and working within the Charitys HR policies on all staffing matters, including sickness, capability and disciplinary issues seeking guidance from the HR department as required. 13. Work with the ward sister to prioritise staff retention and using all available strategies to retain and motivate staff.
14. To work alongside the ward sister to ensure all ward staff have a minimum of an annual appraisal, overseeing the process to ensure all objectives are achievable and contribute to the development of staff and ward objectives. 15. Actively participate in the delivery of training programmes for the clinical team.
16. Ensure that staff keep up to date with mandatory and statutory training requirement in line with their specific job role. 17. Always adhere to the Charitys policies and procedures.
18. To have an awareness of ensuring the skill mix within the ward is appropriate 19. Actively participate with team meetings on a regular basis, recording and circulating minutes of these. Ensure all relevant information is cascades to the unit staff in a timely way.
20. Encourage staff to contribute to the unit development, giving time and opportunity to produce and develop ideas and new ways of working. 21. Work alongside the senior nurses in the fellow hospice to ensure, where possible, there is a consistent approach to all aspects of patient support and care.
Management and Personal Development 1. Accountable for your own practice in line with NMC Code, taking every reasonable opportunity to sustain and improve your knowledge and professional competence. 2. Act as a professional nurse leader and role model to the team.
To apply clinical skills of the highest standard and to take a proactive approach to the development of clinical competence 3. Ensure own personal development is supported via competencies and appraisal, working with the ward sister to implement any development in line with service requirements. 4. Maintain own portfolio to support revalidation.
Clinical Governance, Audit and Research 1. Work alongside the ward sister to ensure the highest standards of infection prevention and control are always practised. 2. Be proactive in the risk assessment, management, clinical incident reporting and management of complaints.
Work with the ward sister , clinical governance team and specialist nurse lead to ensure realistic action plans are developed and implemented. 3. Ensure all staff move and handle patients and goods in ways that promote the health and welfare of the patients and care team and are consistent with legislation. 4.
Undertake clinical and associated audits as appropriate for the given area. 5. Participate in the clinical incidence forum and clinical governance group, presenting to the group as required. 6.
Work with the ward sister, clinical governance and patient safety leads to ensure that all patient, family and professional feedback is used to improve care.
