The role
We are looking for a Senior Social Care Assessor to work within the County team in Herefordshire County Council. The County team work with adults with complex needs, this includes people living with Learning Difficulties/Disabilities, Mental Health, Forensic and/or complex health and social care needs.
The role is hybrid working at home and the office base which is Plough Lane, Hereford. The role demands that you drive and have your own vehicle as there is an expectation that you will travel across the County.
As a Senior Social Care Assessor you will work as part of a wider teams within the Directorate, partners, social care assessors and social workers with scope to practice with a diverse group of adults and informal carers; undertaking strengths based assessments, mental capacity assessments, care management, and reviews. You will be committed to continuing professional development and in return will be supported by training and professional development opportunities and reflective supervision.
About You

As a Senior Social Care Assessor you will have:

  • An NVQ3 qualification or above in related field, or equivalent qualification/experience. GCSE or Equivalent in English and Maths.
  • Experience and an understanding of Adult Social Care including the statutory duties and principles to support vulnerable Adults and Carers in meeting their eligible outcomes.
  • An awareness and understanding of current legislation including Care Act 2014, Mental Capacity Act 2005, Human Rights Act 1998, Equality Act 2010, Mental Health Act 1983/2007, Data Protection Act.
  • Excellent IT skills.
  • An ability to develop and maintain good working relationships with service users, their families, carers, advocates and stakeholders.
  • Willingness to progress in your continuous professional development through independent learning, reflective practice and training opportunities.
