Description:
The NUHS@Home nurse provides nursing care to patients who require acute medical treatment but who are able to be cared for in their homes rather than in a hospital. The nurse works in collaboration with physicians and other healthcare professionals to develop and implement care plans, monitor patient progress, and educate patients and their families on self-care techniques.
The NUHS@Home nurse is competent in performing care assessment and planning, and in the management of patients with identified care needs. S/He provides clinical supervision, precepts the junior nurses and contributes to NUHS@Home nursing training.
JOB SPECIFICATION/REQUIREMENTS
- Possess Diploma / Degree in Nursing and must be registered with Singapore Nursing Board
- Registered Nurse with minimum of 3-5 years of post-registration clinical experience
- Ability to work independently and as a team
- Strong organisational, coordination, interpersonal and communication skills
- Ability to work a flexible schedule, including evenings, weekends and public holidays
Responsibilities:
Primary Responsibilities and Duties (80%)
- Conduct patient assessments and develop individualized care plans
- Administer medications, treatments and procedures as ordered by the physician
- Monitor and evaluate patient progress and adjust care plans as needed
- Communicate with physicians, other healthcare professionals, patients and families regarding patient care
- Educate patients and families on self-care techniques and disease management
- Ensure patient safety and infection control measures are maintained
- Document patient care and progress in electronic medical records
- Participate in quality improvement initiatives
- Adhere to ethical and legal standards of nursing practice
- Utilise technology in care delivery
Secondary Responsibilities and Duties (20%)
- Participates in research, clinical or service quality activities
- Demonstrates awareness of current nursing and health care practices
- Assumes additional responsibilities as assigned by Nursing Officers, Senior Nursing Officers, Assistant/Deputy Director (Nursing) and Chief Nurse
