It’s an exciting time for our award winning Urgent Community Response (UCR) Service. We are expanding our team to include the ability to provide virtual ward capacity whilst continuing to build on our successful UCR model.
UCR services work with patients in their homes across Stockport community to avoid hospital admissions, support those being discharged home and provide virtual ward support using remote monitoring technology and enhanced medical oversight.
We are looking for Senior Support Workers to join us, so if you’re an enthusiastic and motivated individual looking to be part of a team providing excellent cutting-edge urgent community care, we’d love to hear from you.
We are a multidisciplinary team working flexibly to provide a seven-day service, 365 days a year, between 8am and 10pm. We deliver:
A rapid response to people in the community who are experiencing acute episodes of ill-health.
Virtual ward admission utilising remote monitoring technology and enhanced medical oversight to provide a safe alternative to hospital admission.

ESSENTIAL CAR USER AND UK DRIVING LICENCE HOLDERS ONLY WILL BE CONSIDERED.

The focus of this role is providing personal, health and therapy care for patients across the area with acute and diverse needs.
Your day will be varied and interesting, supporting patients towards achieving more independence in their activities of daily living. You will be supported by nursing, physiotherapy, and occupational therapy colleagues to review the needs of patients and input into onward referrals.
Your role will include assisting in the ongoing assessment and provision of care, reablement and reporting all changes in patient status to your registered colleagues both verbally and in written records. You will undertake simple clinical and therapeutic procedures such as taking physical observations, venepuncture, administering medications, simple wound care, rehabilitation programmes and equipment provision. You will record the outcome of contacts with patients, their relatives, and carers on to the electronic patient record. You will have a key role in empowering patients to regain their health and independence in collaboration with the multidisciplinary team.

  Work within the Urgent Community Response Team, providing high quality, caring, compassionate care to patients and their families/carers. To include care as directed by UCR clinician following bespoke care plan.
  Work under the direction of the Registered Healthcare professionals and senior colleagues, both independently and as part of the multi- disciplinary team.
  Under the direction of a Registered Health Care Professional ensure a high standard of service provision for patients in community, by practicing effective clinical and administrative preparation, effective communication and care delivery, prioritising workload appropriately and contributing to service improvements
  Participate in giving compassionate high quality care to patients prioritising appropriately and developing competencies that safeguard practice. Develop competencies appropriate to the role.
  Act always in a manner, which illustrates respect for privacy, dignity, equality, diversity, and confidentiality.
  Exhibit professional attitude and behaviour and always demonstrate excellent customer care skills.
  Contribute to effective team working by being fully involved, committed, and participating, to achieving own and others full potential.
  The role will include visiting patients in their own homes undertaking lone working without direct supervision.
  The scope of the role will focus on supporting admission avoidance, early supported discharge, promotion of independence and assisting in the effective management of patients whilst on the Urgent Community Response caseload.
  Will be required to cover other Band 3 work within the neighbourhood team or Stockport borough during periods of annual leave, sickness and maternity leave as appropriate for the maintenance of the service.

  If successful this vacancy will require an enhanced DBS check at a cost of £41.90. This cost will be deducted from your first Trust salary
  In addition to this it will be a condition of your employment that you will join the DBS update service and pay the annual subscription fee of £13
