Description:

Are you passionate about supporting frail patients in maintaining independence and maximizing health outcomes? Do you thrive in fast-paced environments and have a strong commitment to patient-centered care?
We are looking for a dedicated and dynamic Occupational Therapist or Physiotherapist to join our Urgent and Emergency Care Therapy Team for a 5-month position, working within our new Frailty Unit.
In this role, you’ll work as part of our Frailty Intervention Team, where you will:
Assess, treat, and support frail patients, helping them achieve and maintain independence.
Contribute to preventing hospital admissions and supporting complex discharges.
Collaborate across multiple units, including the Emergency Department, Emergency Assessment Unit, and Short Stay Ward.
Provide supervision and mentorship to Band 5 OTs and Physios, support student education, and assist in managing the daily operations of the team.
Engage in service development projects aimed at improving patient care and team effectiveness.
