Responsibilities:

We are seeking a Services Coordinator (Disability) to join our NDIS team on a full-time basis, who have experience in overseeing the delivery of services to clients with a disability, who have complex care needs. This position will identify client’s care needs, undertake risk assessments, complete support plans and identify training needs for their staff teams. This role would also provide Support Coordination for our Veterans Home Care Services.

In this role, you will be responsible for coordinating, monitoring and reviewing disability services for new and existing NDIS and VHC clients by:

  • Coordinating person-centered supports for people living with a disability, helping them reach their goals and maintain their independence.
  • Ensuring client needs are understood and that your team have the skills and knowledge they need to meet them
  • Collaborating with clients, families, allied health professionals and other stakeholders to develop support plans for clients and monitor services throughout the NDIS plan
  • Conducting comprehensive assessments with clients and their support teams to capture clients’ clinical and social support needs
  • Conducting regular client reviews and maintain client documents.
  • Ensuring clear and up to date client records are maintained, to support quality service provision
  • Monitor client budgets as per Service Agreements in consultation with client and families
  • Providing supervision and guidance to a team of support workers on a daily basis
  • Ensuring clients rights are met and supporting them to exercise choice and control
  • Conducting interviews for support workers for your team and mentor your support team

Essential:

  • Experience working with people living with disability
  • Demonstrated ability to provide outcome based, person-centered supports
  • Excellent communication skills and the ability to adapt your style to suit the needs of the person or situation
  • Excellent listening skills
  • Strong administrative and organisational skills with a high level of attention to detail
  • A sensitivity to cultural diversity
  • Demonstrated experience in overseeing high-volume of support services while maintaining quality of support
  • The right to work in Australia
  • A current Western Australian driver’s licence
  • National Police Clearance (not older than 6 months) and NDIS Worker Screening Check.

  • A Nursing Qualification - Enrolled Nurse or Registered Nurse

Desirable:

  • Diploma or higher in Disability/Individual Support or Community Services
  • Experience supervising a team of support workers in the community
  • Experience with scheduling or rostering services in Disability Services environment
  • A comprehensive knowledge of the NDIS sector
  • Previous experience as an NDIS Support Coordinator
