Description:

Job Purpose The Service Manager (SM) for the learning disability and autism outreach service will work as an integral part of the Learning Disability Management Team. The post holder will be responsible for ensuring the operational and performance management of the learning disability and autism outreach service. They will ensure that the work of clinical staff is to the highest standards of evidence based practice, implement quality and safety standards and ensure compliance with commissioning standards, and the development of clinical pathways. The SM will ensure that clinical process takes place in line with practice standards, this will include assessment, planning, implementation and review and that robust and consistent risk management processes are in place.
They will be responsible for the management of resources to deliver outreach services which meet the required financial, performance and quality standards and ensure that the Directorate governance and assurance systems are in place, They will provide day to day leadership and management to the clinical team Duties and Responsibilities Communication and Working Relationship Skills Ensure effective multi-disciplinary team communication and facilitate a multi-professional and multi-agency approach Establish and maintain effective working relationships with both internal and external stakeholders, being an active member of stakeholder/systems groups as required. Promoting understanding of learning disability and autistic spectrum conditions within the wider health & social care pathway The Service Manager will be required to provide and receive highly complex/sensitive information and data which they will need to share with a wide range of stakeholders. The post holder will require developed persuasive and motivational skills. The Service Manager should be able to demonstrate good communication, written, verbal and presentation skills.
Also the post holder should be able to demonstrate good negotiating skills in what may be difficult, sometimes confrontational, situations. Role model excellent clinical practice. Be visible, accessible and approachable to all people who use and work within the service. To work with other service leads to facilitate business, governance and clinical meetings within the service Analytical and Judgemental Skills They will be required to analyse and interpret a wide range of information from a variety of sources and design, implement and monitor action plans based on the analysis, including within the framework of PBS and other identified NICE guidance.
The management of risk is an integral part of the role with decisions needing to be made on risk assessment basis. They will be required to analyse and make judgements on completing and conflicting demands and formulate actions to ensure that the service is delivered and resourced effectively. To undertake disciplinary investigations as required. To undertake and chair risk management and safeguarding meetings as required.
To undertake review of complaints and manage these in a sensitive manner. Planning and Organisational Skills To participate in Trust wide projects, development and change processes as required The Service Manager has joint responsibility for convening and leading meetings and initiatives to ensure provision of coherent and well managed local delivery of care. The Service Manager will be required to jointly plan and organise a range of activities in support of the implementation of the service model. This will require the formulation, implementation and monitoring of the plan.
Ensure compliance with commissioning standards, , and the development of clinical pathways To ensure that the whole workforce are efficiently deployed to deliver services safely and effectively through workforce planning and development On a day to day basis the Service Manager will- plan and organise the work of the team including staff training and development To develop, monitor and maintain effective staff rostering systems, ensuring Effective use of available resources to maintain safety and ensure that this is within budget Implement and evaluate systems to monitor risk Responsibility for Patient/Client Care, Treatment and Therapy Ensure that the work of the clinical staff is to the highest standards of evidence based practice. Provide daily leadership of the implementation of the outreach clinical pathways within the community setting in conjunction with clinicians. To ensure holistic assessment and treatment provision including risk assessment, care planning and review. Effective monitoring and quality assurance process must be developed and acted upon.
To be responsible for the delivery of the highest standards of patient care and treatment in keeping with standards set out in NICE Guidelines/National Documents. In partnership with the multi-disciplinary team determine a therapeutic programme of care that is evidence based and meets the needs of the people who use our services Ensure systems are in place that promote recovery, health and well-being Ensure that the physical health needs of people using services are assessed and appropriately supported and appropriate treatment plans are in place and regularly reviewed. To ensure that care is delivered with privacy dignity and respect. Lead on the development of clinical and organisational care pathways The Service Manager is responsible for a range of developments that support improvement in standards of care working closely with all clinicians to develop effective care pathways in line with evidence based practice.
The Service Manager will have responsibility for assuring governance in line with the Specialist Services directorate assurance framework (Including CQC compliance). Participate in Safeguarding Adults and Children to Level 3 as per the Trust policy Ensure services are compliant with Infection prevention and control best practice guidance (this including the hygiene code) To ensure that the role of the carer is understood, considered and acknowledged within the care of the patient Responsibility for Policy and Service Development Implementation Working as an integral part of the Learning Disability Management Team to: Implement and monitor clinical and operational policy Develop and strengthen communication processes internally and externally Implement performance management arrangements. Implement quality and safety arrangements Work with staff to embed a recovery and/or wellness focus. Ensure compliance with commissioning standards, and the development of clinical pathways The Service Manager will be required to lead, advise and support the planning and implementation of a range of new ways of working and service models.
The Service Manager will develop and implement policies for the directorate and ensure implementation through the Learning Disability and autism services. Ensure that the outcomes of service delivery are measured and improvement plans are developed as part of the Learning Disability and autism strategy With Professional Leads develop a PBS approach to service delivery across the outreach service. The Service Manager will be required to analyse and interpret competing and conflicting demands on services to determine the effective use of physical and financial resources. This will involve monitoring performance indicators and trends so resources are effectively managed/allocated.
The Service Manager will be an authorised signatory for pay and non pay expenditure and will ensure that controls are in place for budgets that are devolved or delegated to others
