JOB OVERVIEW:
The RWJBH Social Impact and Community Investment or SICI SNAP Navigator will assist patients and community members with screening, providing eligibility information and enrollment regarding SNAP or Supplemental Nutrition Assistance Program benefits.
The Navigator will provide services in RWJBH clinical spaces, outreach events, and with community partner organizations in the county/counties in which they are assigned. This role will operate in alignment with RWJBH’s commitment to combatting food insecurity in the communities we serve.
We are looking for an enthusiastic team member who is passionate about making healthy food available for our neighbors in need.
QUALIFICATIONS:
Required:
- This position requires excellent organizational and interpersonal skills.
- Solution-driven, self-motivated, empathetic.
- Comprehension of food security/food access issues.
- Proficiency in Microsoft Office Suite.
- Valid driver’s license and vehicle; automobile/vehicle with valid insurance.
Preferred:
- Bilingual Spanish or Haitian Creole speaking is preferred.
- Individuals with experience in Public and/or Community Health are best suited for this job, however relevant work experience in benefits enrollment programs will also be considered.

SCHEDULING REQUIREMENTS:
- Full-time, Day
- This position will sit at Jersey City Medical Center.
- Flexibility to work event-based scheduling, including occasional evenings and weekends.
Responsibilities:
ESSENTIAL FUNCTIONS:
- Receive referrals from clinical and non-clinical staff, community partners and through direct inquiries to screen and enroll individuals in SNAP benefits.
- Assist in completion of new and renewal benefits applications on behalf of patients/community members.
- Provide follow-up assistance regarding status updates on completed applications.
- Coordinate scheduling with community partner organizations to provide services.
- Maintain and foster new relationships with partner organizations.
- Participating in recurring state and partner organization led training to reach and maintain expertise in program eligibility requirements and application procedures.
- Collect enrollment and additional data for monthly departmental and quarterly state reporting.
- The SNAP Navigator will report to the SNAP Navigator Program Manager, who reports to the SVP of Social Impact and Community Investment.
