Description:

Over the last 4 years we have been working to improve the support provided to adults over 18 who are experiencing multiple disadvantages due to homelessness, substance misuse, domestic abuse and sexual exploitation.
The team consists of x3 Social Care Workers, you will receive monthly supervision with the Safeguarding Manager, have strong connections with the safeguarding team for regular Team Meetings, peer supervision and access to relevant training.
Your role will be based on an assertive outreach model, working to support and engage adults. This will involve offering practical advice and support to access services in addition to gathering relevant assessment information and developing a relationship of trust with the individual.
