About the role

Carry out a defined range of tasks as prescribed by the social worker and work under their general direction, including:

• Fulfill the responsibilities of this role in accordance with the Metro North Hospital and Health Service values.
• Work cooperatively and effectively within a social work team environment, as well as with patients/clients and community partners
• Completion and or submission of forms and other paperwork e.g. At the direction of social workers, to assist patients and families with paperwork related to applications for housing, Centrelink, residential care, QCAT etc
• Liaison with external providers including residential care facilities, community organisations, government, and non- government services
