Responsibilities:
- Respond to referrals from Care Coordinators to appropriately administer social work services to patients.
- Assess patients to identify and determine psychosocial needs and therapeutic treatment needs and goals.
- Provide treatment program, counseling strategies, and emotional/mental health support to patients.
- Oversee patient’s progress and adjust treatment programs when needed, to ensure therapy goals are met.
- Educate and counsel patients, their families, caregivers, and/or SDMs regarding issues, challenges and strategies for continuous improvement.
- Accurately document and chart patient’s assessment results, goals, treatment plans, and interventions.
- Link patients with other in-house services or outside organizations, if needed.
- Balance patient needs and choices with available resources, ensuring their values and preferences are respected.
