As a Social Worker (Temporary, Full-Time, effective immediately to July 18, 2025) you are responsible for the provision of social work services to patients, in order to achieve optimum psychosocial functioning, prevent unnecessary deterioration, and promote patient independence.

Responsibilities:

  • Respond to referrals from Care Coordinators to appropriately administer social work services to patients.
  • Assess patients to identify and determine psychosocial needs and therapeutic treatment needs and goals.
  • Provide treatment program, counseling strategies, and emotional/mental health support to patients.
  • Oversee patient’s progress and adjust treatment programs when needed, to ensure therapy goals are met.
  • Educate and counsel patients, their families, caregivers, and/or SDMs regarding issues, challenges and strategies for continuous improvement.
  • Accurately document and chart patient’s assessment results, goals, treatment plans, and interventions.
  • Link patients with other in-house services or outside organizations, if needed.
  • Balance patient needs and choices with available resources, ensuring their values and preferences are respected.
