Job Description
The Complex Care and Recovery Program is currently seeking a full-time, temporary (18 months) Social Worker 2 for the Downtown West Clinic Outpatient team. Reporting to the Manager, you will be a member of an active outpatient team member working with people with severe and persistent mental illness. The Complex Care and Recovery Program is client centered and treatment is based on the principles of recovery for adults with major mental illness, predominantly Schizophrenia and Concurrent Disorders. As a member of an inter-professional team, you will participate in the assessment and treatment process needed to proactively engage clients in their recovery goals. You will be responsible for conducting psychosocial-based assessments, providing financial and social supports/assistance and counseling to clients, family and caregivers from diverse backgrounds. You will assume a role in the client’s psychosocial activities – particularly in addressing the social determinants of health. You will have sound knowledge of schizophrenia, Schizoaffective disorders as well as demonstrated familiarity with best practices in the assessment and treatment of this illness to include concurrent disorders Screening and recovery principles. Other duties will include formulating treatment plans, completing standardized assessments, motivational interviewing, Relapse prevention, Concurrent Disorders Case-management, Concurrent Disorders group programming, monitoring client’s response to therapeutic interventions, and communicating with the client’s social and community networks. Knowledge of relevant community resources and working with families/significant others is required. Using your excellent interpersonal skills, you will network with other referral sources in coordinating a smooth flow of clients within the various services of the program. You will liaise with community providers and participate in the education of clients and their families, and mental health providers both within and outside the Centre. Candidates must be capable of providing supervision of Masters of Social Work students, collaborating with faculties of social work on special projects related to the development of the social work field, contributing to the development of best practices unique to the social work field and also teaching to other professionals. This position involves shift work, some evening and weekend work, and will be primarily in the community providing FACT level of service. You will support a workplace that embraces diversity, encourages teamwork and complies with all applicable and regulatory requirements. This position is based at 10 White Squirrel Way but will involve significant time and travel in the community.
Job Requirements
The successful candidate will possess a Master’s of Social Work degree combined with a preferred 3 years of post-graduate work experience and be registered and a member in good standing with the Ontario College of Social Workers and Social Service Workers. The Candidate must be eligible for an adjunct faculty appointment with the faculties of social work that has affiliation agreements with CAMH. Knowledge of relevant community resources and working with families/significant others is required. You will demonstrate knowledge of mental health and substance use issues. Outpatient experience in a mental health setting is an asset. Excellent organizational, interpersonal, written communication skills and the ability to work effectively in a fast-paced clinical environment are essential. You possess excellent organizational, communication and interpersonal skills. You will be expected to participate in research, education and supervision of MSW students. The ability to work effectively as a team member in a dynamic inter-professional environment delivering the Flexible Assertive Community Treatment Model (FACT) is essential. You possess demonstrated knowledge and experience in assessment and rehabilitation. In addition, you possess fundamental knowledge and application of the recovery model and other current Best Practices for the Complex Mental Illness Program clients. Experience working with clients, staff and organizations of diverse ethno racial backgrounds is required. Computer skills, including word processing, electronic communication, and workload measures are essential. Bilingualism (French/English) or proficiency in a second language would be an asset.
