POSITION DESCRIPTION
The Social Worker will work collaboratively with the Clinical Leads for the Substance Use Program and the Transgender Youth Clinic (TYC). The role involves providing social work services, including acting as a therapist for caregivers of teens with substance use disorders and providing services through in-depth psychosocial assessments and interventions for youth and caregivers in the TYC.
Responsibilities:
- Conduct psychosocial assessments working in collaboration with other members of the program team, with adolescents and caregivers of adolescents who have alcohol and substance use disorders or are affected by substance use, and who are attending individual and/or day treatment services in the Substance Use Program.
- Develop and implement treatment plans which build coping skills, improve relationships, and quality of life. Such treatment would be conducted through a variety of modalities including Acceptance and Commitment Therapy (ACT), DBT, CBT, MI, and others.
- Assume responsibility for all aspects of social work practice including issues of productivity and utilization.
- Collaborate/communicate with hospital health care teams and community agencies.
- Practice in a manner that is consistent with the philosophy of continuous quality improvement and evidence-based practice.
- Provide therapeutic support including supportive therapy and mental health counseling to adolescents and their caregivers with a variety of substance use and concurrent disorders (co-existing substance use and mental health disorders).
- Provide services through in-depth psychosocial assessments and interventions for youth and caregivers in the Transgender Youth Clinic (TYC).
- Conduct psychosocial assessments, develop and implement intervention plans, and provide brief intervention and supportive counseling.
