This is an exciting opportunity for a Social Worker II in the Adult Inpatient Program at the Centennial Centre for Mental Health and Brain Injury (CCMHBI) in Ponoka. CCMHBI is a campus of specialists and health professionals that are committed to integrated, multi-disciplinary care and treatment to ensure the best outcomes for patients and their families. This Social Worker II - AIP position is based on Bryce/Logan House and requires a highly motivated individual that is keenly interested in working with patients within a concurrent disorders environment. Strong interpersonal, organizational and communications skills are large contributors to success in this position. You will be reporting to a Unit Manager and will be an active member of the interdisciplinary team. Located in the community of Ponoka, within the parkland region of central Alberta, the Centennial Centre is one of the most modern mental health referral centers in Western Canada. This position has transitioned to Recovery Alberta effective September 1, 2024. Description:
As a Social Worker I, you will be a member of a multi- disciplinary team that provides mental health services to individuals, families and/or groups at a community based facility. You will be responsible for the initial screening of patients by gathering and documenting client information, basic assessment and providing support to clients and their families’ short-term needs. You will liaise with and refer clients to appropriate community and government services and support, along with participating in case conferences and team meetings.
