Description:
Job Overview:
The Social Worker administers individual counseling to Caregivers who meet the specified criteria under the Title III E Caregiver Grant. Facilitate the monthly Caregiver Support Group.
QUALIFICATIONS:
Required:
- LCSW with at least 1 year experience in health care OR
- LSW under the supervision of an LCSW
SCHEDULING REQUIREMENTS:
- Day Shift
- Part-Time
- 10-15 hours per week
Responsibilities:
- Assesses and evaluates each caregiver’s personal history and that of the person being cared for
- Individualizes a care plan based on evaluation of all aspects of a client’s current needs for support and referral to appropriate agencies for assistance
- Promotes program services to all health care providers in Ocean County
- Properly records all elements of visit on client s chart
- Researches new resources for caregivers in Ocean County
- Facilitates weekly Caregiver’s Support Group
- Counseling to develop and enhance coping skills, communication techniques, and advocacy skills
- Counseling for care planning and decision-making
- Performs other duties as assigned
