About the role
Working as part of a comprehensive multidisciplinary team, you will provide leave cover for the social work team across the inpatient, outpatient and home-based programs, including the commonwealth home support program and carer support program at Calvary Kooyong Bethlehem Hospital. Social work duties include, psychosocial assessment and provision of social work intervention including, assistance with financial and accommodation issues, adjustment to diagnosis, advance care planning, referral to community agencies, carer support, and family violence risk assessment and intervention. Possibility for two casual positions or a combined role:
Flexible casual bank - leave cover: variable hours, variable roster (weekdays only) - immediate start
Short-term project work (flexible hours per week)
Responsibilities:
Psychosocial assessment and provision of social work intervention including, assistance with financial and accommodation issues, adjustment to diagnosis, advance care planning, referral to community agencies, carer support, and family violence risk assessment and intervention.
About you
Experience in hospital social work, palliative care, progressive neurology and/or aged care
Strong knowledge of systems and supports that help people live well with advanced illness and/or aging
Experience in working with the MyAgedCare and/or NDIS
Ability to work autonomously and as part of a team
Tertiary qualification in social work
Eligible for registration with the Australian Association of Social Work (AASW)
A desire to join a team committed to the care of others in a mission based organisation
Interest and/or experience in project work desirable for project casual.
