The Brant Community Healthcare System is a two site Community Hospital located in Brantford and Paris, Ontario.

- The Willett in Paris is an urgent care centre and transitional beds

EDUCATION REQUIREMENTS
- Master of Social Work Degree (M.S.W.) from an accredited University School of Social Work (preferred)
- Registration with the Ontario College of Social Work and Social Service Workers (OCSWSSW)
MANDATORY QUALIFICATIONS
- Recent Social Work experience in a hospital, including acute Medical and post-acute programs and discharge planning for populations listed above.
- Certification and additional training in the areas of End-of-life care, Grief and Loss and supporting loved ones.
- Certification and training in Gentle Persuasive Approaches and working with Cognitive Impairment
- Demonstrated competency in providing services using an anti-oppressive approach as a member of an interdisciplinary team.
- Completion of San’yas Indigenous Cultural Safety Training
- Proven ability to manage competing demands and prioritize work in a time-sensitive environment.
- Excellent knowledge of the Home First philosophy, Alternate Level of Care (ALC) legislation and processes
- Excellent working knowledge of related provincial legislation including Health Care Consent Act, Substitute Decisions Act, Public Hospitals Act, community resources
- Working knowledge of social supports including, Ontario Works, Ontario Disability Support Program (ODSP).
- Knowledge of psychosocial, relational and health issues across the life span, including family birthing, paediatrics, grief and loss, caregiver stress/burnout, chronic illness and psychosocial/physiological conditions affecting the patient population
- Proven ability to work in an empathic manner to engage hard to reach and marginalized populations
- Evidence of ongoing professional development with respect to the practice of social work
- Excellent written, verbal and interpersonal skills
- Excellent attendance record
