Care Corner Singapore Ltd was founded in 1981, in a climate of changes in the early 1980s, as economic competition in the region intensified and the rift between the wealthy and the lower-income in particular the Chinese blue-collar workers, widened. Rev Tan Tien Ser and a group of passionate individuals believed that these marginalised people with lower income and little opportunities for a formal education could be helped to realise their potential and become more competent and fulfilled persons. Through a series of early outreach initiatives, Care Corner has evolved into an organisation offering a wide range of programmes and services delivered through various social service centres located island-wide.In a bid to stay relevant to the needs of the community, Care Corner has continuously strived to offer new and innovative programmes to support those in need and the less privileged.
DESIGNATION : Social Worker/Executive (Community Case Management Service)
QUALIFICATIONS
Bachelor in Social Work or relevant fields (Psychology, Community Work); candidates from non-social work qualification will be considered as Social Work Executive.
REQUIREMENTS:
- At least 2-3 years of relevant working experience; knowledge and experience in eldercare/healthcare sector will be advantageous
- High proficiency in written and spoken English; able to speak in mandarin and/or dialects would be essential for engagement with seniors
- Passionate to work with seniors and caregivers
- Strong interpersonal and communication abilities with individuals and systems at multi-levels
- Good analytical and assessment skills
- Team player with an ability to function effectively in a fast-paced environment
- Strong organisational skills and structured.
Responsibilities:
You will be part of the Community Case Management Service team, providing support to vulnerable seniors and their caregivers who face difficulties in managing their multiple health and social care needs. Key responsibilities:
- Provide intake and triage services for members of the community;
- Casework, counselling and service coordination for seniors and caregivers;
- Conduct psychosocial assessment and coordinate interventions with healthcare and community professionals to support seniors and caregivers;
- Document and review case, interventions and outcomes within set timelines, ensuring case documentation submitted on a timely manner
- Liaise and work with organisations, partners and volunteers to build an ecosystem of assets and resources to facilitate more effective work with seniors and caregivers.
- Assist with report submissions or other projects, as assigned by Manager
- Opportunities to be involved in organisational level projects.
