Salary: Range SW2 - £35,935 - £42,625 per annum
Location: Hybrid – Works part in the office (Gun Wharf) and part from home
Contract: Permanent

We are currently recruiting for a Social Worker within our integrated discharge team to contribute to the delivery of highly effective children and adult services (within Adult Social Care) in an efficient manner. The service is delivered using a strength based, personalised approach.
We work across both the acute and non-acute sites across Medway and out of area hospitals. We have been at the forefront of changes in the hospitals discharge to assess model. This has meant that our teamwork with people in an acute setting as well as those in community settings after a period of the time in hospital. This can either be in a health bed, social care step down bed or in the persons’ own home. This role also includes a role in the emergency department of the acute hospital by working with our health colleagues.
To work closely with colleagues within the Council and partners ensuring that person centred outcomes are met in ways which utilises available resources as effectively as possible.
To work within organisational policy and legislative requirements including The Care Act, Mental Capacity Act and Mental health Act.
