Description:
Your Opportunity:
An integral member of the Awasisak Indigenous Health team and reporting to the Awasisak Team Lead, and the Social Work Program manager the SWII – AIH collaborates with the multidisciplinary patient care team, providing liaison and case coordination for Indigenous patients and families at the Stollery Children’s Hospital. The incumbent will be responsible for ensuring the health care needs of Indigenous patients are met through appropriate case management, patient advocacy, and discharge planning activities as specified. The SWII – AIH also acts as a single point of contact between families and health care providers in hospital and in community. These services may include; psychosocial assessments; resource-based interventions; risk screening; counselling; case management & coordination; transition planning; prevention & promotion and public awareness activities. This position will also be responsible for building bridges with Indigenous First Nations and Metis community health centers and agencies in central to northern Alberta, other health organizations, Stollery Children’s Hospital Foundation, and Ronald McDonald House is essential. Facilitating in-services for staff and attending seminars on various topics; consultation with Traditional Elders for support and guidance is encouraged; communicates and models expectations for families while their child is a patient in a Patient-Family-Centered Care environment. The incumbent must be able to work independently with a high degree of judgment and must be able to work effectively as part of an interprofessional team and commit to strengthening dynamics by actively facilitating interprofessional collaboration to enhance patient health outcomes.
