Your Opportunity:

The SHC has four key pillars of service: Collaborative Practice, Wellness, Patient and Family Centered Care, and Innovation. These pillars are guiding the development and delivery of service to all patients and families at SHC. We are looking for a collaborative and innovative Social Worker II to join our multidisciplinary team and provide direct patent care to the Emergency Department. This position also provides coverage to Inpatient Medicine units, as required. Under the supervision of the Allied Health Assistant Manager and support from the Clinical Practice Leader, the SWII position will work within the established standards, directives and policies for the work environment and the profession. The successful incumbent will demonstrate strong interpersonal skills with the ability to maintain collaborative working relationships. They will demonstrate sound clinical judgement and critical thinking skills, as well as have strong time management and prioritization skills. The present caseload for this position is in the Emergency Department on a day, evening and weekend rotation. The caseload and work hours for all positions within the Allied Health department may be subject to change based on operational considerations.
Description:

As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.
