Working as part of an interdisciplinary team within the Adult Acute portfolio, the SW will provided the opportunity to advocate for those in need of mental health support. We provide patient and family centered care, with all team members working within the CoACT collaborative care model and the Addiction and Mental Health - Edmonton Zone Principles of Care. Reporting to the Unit Manager for unit 4G2, the Social Worker is part of an interdisciplinary team that facilitates the recovery of Addiction & Mental Health patients by assisting with their psychosocial needs while in the Acute Adult Mental Health Program based at the UAH. Duties include assessment, education, treatment, advocacy, resource finding, consultation and discharge planning for patients and their families. The incumbent will also participate in non-patient care activities such as program planning, evaluation, quality improvement, research and staff development, along with other duties as required by the Unit Manager. This position has been identified and is anticipated to transition to Recovery Alberta effective September 1, 2024.
