Your Opportunity:
The Alberta Children’s Hospital (ACH) is a warm & vibrant workplace, with team members providing service to children & their families in inpatient units & ambulatory clinics. The Social Worker III, as part of the ACH site-wide Social Work Service, ensures their clinical practice is patient/family-centred, delivered by the Social Work Standards of Practice and Code of Ethics, evidence-based and research-informed and aligned with program/clinic frameworks. The Social Worker III in this role will deliver primary support to the Hematology, Oncology, Transplant, and Immunology (HOTI) clinics in collaboration with 3 other Social Workers and alongside a comprehensive interdisciplinary team. The Social Worker will integrate and synthesize multiple levels of information in adherence to evidence-based practice, professional standards and applicable legislation, policy, and guidelines. Social work services will include triage, intake, psychosocial assessment, specialized resource intervention; risk screening, group facilitation; family education, supportive counselling, discharge planning and consultation with other health care providers. The role requires advanced critical thinking, professional judgment and the ability to conceptualize and manage complex cases from a range of diverse perspectives to facilitate agreed-upon outcomes. As the successful candidate, you will demonstrate the ability to work collaboratively within an interdisciplinary team; will exhibit excellent clinical reasoning & problem-solving skills; will be organized in managing a diverse & complex caseload; will demonstrate initiative, adaptability & flexibility to meet varying clinical & operational needs; will demonstrate effective interpersonal & verbal/written communication skills. Supervision of students is required. Participation in program/clinic committees is expected.
Description:
As a Social Worker III, you will perform either advanced level assessment and treatment or act as a clinical lead. You will work with or within teams to provide health care services for patients with complex and multidimensional medical, psychosocial, environmental and instrumental needs. You will provide advanced clinical services, consultation to the interprofessional team and patients/families and supervision of students and team members as assigned. You may also act as a clinical lead providing leadership in the planning, development, and delivery of services in a specific area, as well as staff development, quality improvement and evaluation or research activities as identified by the needs of the service area(s).
