Wage Schedule: $44.670 - $57.900 per hour

QUALIFICATIONS:
- Master’s degree in Social Work;
- Registration with the OCSWSSW;
- Nephrology Program social work experience is required;
- Demonstrated experience with psychological assessments, complex discharge planning, crisis intervention and adjustment counselling;
- Excellent counselling skills and knowledge of community support services; demonstrated knowledge of, and commitment to the principles of chronic disease prevention and management;
- Excellent knowledge of, and experience within the health care system, particularly related to complex diabetes conditions;
- Experience within a clinic-based setting an asset;
- Demonstrated exceptional interpersonal, verbal and written communication skills and judgement when relating to adult clients, families, co-workers and members of the interdisciplinary team;
- Demonstrated ability to build and maintain effective relationships with a diverse range of people and work effectively within an inter-professional team;
- Demonstrated ability to organize workload and establish priorities effectively;
- Demonstrated problem solving skills and ability to exercise independent judgement and to assess situations and address problems efficiently and effectively;
- Motivated self-directed learner, who takes initiative and seeks learning opportunities to enhance client care and professional competence
- Demonstrated understanding of and commitment to comprehensive safety programs and practices including staff, client and environmental safety and participation in regular in-service education in this area;
- Computer literacy, including knowledge of Meditech and computer programs such as Microsoft Office and Internet Explorer an asset;

Responsibilities:
The Renal Social Worker will play a critical role within the LH Nephrology Program, to support our pre-dialysis patients in clinic, provide expertise for our home hemodialysis and peritoneal dialysis programs and provide on-going care to patients in our in-centre hemodialysis units. As one of the renal allied health team members, supported by guidelines and deliverables of the Ontario Renal Network (ORN), the Renal Social Worker has several areas of focus in their provision of care. These include: advocating on behalf of patients for community resources, supporting comprehensive conservative renal care planning, developing goal oriented treatment plans and interventions, discussing power of attorney, arranging resources and supports from various foundations, providing patient/family education and support to address psychosocial, emotional and financial needs and supporting advance care planning process.
