Description:

JOB OVERVIEW:

The Licensed Social Worker provides support to the dialysis unit with psychosocial services. As an LSW in out-patient Hemodialysis, you will provide patient assessment/care planning counseling, provide education to patients and staff, have knowledge of insurance and financial assistance and work independently and with an interdisciplinary team.

Our physicians and staff offer years of experience and operate out of three locations that are convenient to area residents. We emphasize expert diagnosis, treatment and early intervention for those at greatest risk.
To better serve our patients, all three Trinitas dialysis facilities have achieved 5-Diamond status through the Quality Insights Renal Network 3. The 5-Diamond Patient Safety Program was developed as an innovative training and recognition program to assist dialysis providers in increasing awareness and building a culture of safety among patients.

QUALIFICATIONS:

Required:

  • Master’s degree in Social Work
  • Basic to intermediate computer skills, MS Office
  • Strong focus on details and delivery of results while maintaining patient and staff satisfaction
  • Strong organizational and time management skills
  • Demonstrated high degree of professional ethics and knowledge of confidential and regulatory compliance practices

Preferred:

  • Previous experience working in a Renal setting

SCHEDULING REQUIREMENTS:

  • Full-time

Responsibilities:

  • Assisting with case management
  • Setting up a care plan
  • Obtaining authorizations from Medicaid, insurances, etc.
  • Other duties as assigned
