Description:
Be responsible for the management of a Specialist Outreach Recovery Team (SORT) caseload. Carry out reviews of caseloads and support the process of recovery and discharge. Promote Recovery To support and participate in assessments into the service. Regularly take a lead in clinical meeting ensuring all patients are reviewed and decisions and plans are put into action and represent the patients views.
Expert ability in receiving information, analysing and formulating professional judgments. Provide clinical supervision to staff contributing to professional development, appraisal, performance review and recruitment. Be familiar with and (alongside the Team Leader) ensure dissemination of local, regional and national policies, procedures and guidelines pertinent to all staff and students within your area of responsibility and comply with their contents in the pursuit of the highest standards of care. Practice within the legal and ethical framework set out by mental health related legislation to ensure that service users interests and wellbeing are at the forefront of practice.
Liaise, advise, develop and maintain good relationships with service users, carers, relatives and fellow health, social care professionals who are involved in the delivery of services. Participate in all activities aimed at keeping professional skills updated and contribute to dissemination of that knowledge and experience to all staff and students within your area of responsibility. Act upon referrals from secondary care by providing assessment, implementation and development of programmes of care to individuals, groups and families. Implement and participate in the formulation of all policy and procedures within the area of responsibility in consultation with relevant others.
Plan, with teaching establishments, a realistic learning experience for students and for postgraduate students and be responsible for the quality of the programme. Participate in and support training programmes designed for all staff groups. Monitor standards of practice within the team, ensuring areas of deficit are addressed Co-ordinate the collation of evidence within the team in support of standards for better health. Practice in an autonomous safe manner to support and encourage engagement with a caseload who are difficult to engage in mental health services.
Maintain close relationship with adult social care colleagues and support the team with adult social care input. The post holder will be an autonomous practitioner who will support the team in referrals from secondary care and be involved in the assessment process. The post holder will supervise caseloads of mental health practitioners within the area of responsibility and review each individuals work. The post holder will frequently review the standards of care provided by the community mental health team with the Team Manager and Team leader e.g.
compliance with Nice Guidance and Developments, PCF. Working alongside the Team leader and Team manager in ensuring that the quality of care is maintained at the highest standard. Balancing demands of own clinical caseload whilst effectively providing supervision and support to a team of experienced practitioners. Assist the Team to manage capacity effectively ensuring throughput so that the people in most need receive an appropriate service.
Lone working and accountability for defined caseload can generate stress however this is mitigated through the provision of line management and clinical supervision
