QUALIFICATIONS:
Required:
- Graduation from an accredited graduate school of social work with an MSW master’s in social work degree.
- State licensure is required by the NJ State board of Social Work Examiners.
- Licensed clinical social worker LCSW or licensed social worker LSW.
- One- or two years experience in a hospital setting preferred.

Responsibilities:
- Provides direct clinical social work services to patients and their families in an interdisciplinary team approach.
- Provide psychosocial assessment and care plans for patients and families.
- Identify barriers to care and develop strategies.
- Provide education for patients and families to ensure adequate age-appropriate knowledge of coagulation disorders, complications, and treatment.
- Maintains updated education library and educational resources for patients/families.
- Coordinate and conduct school visits with patients, parents, and the hemophilia center team.
- Act as a liaison to the school and child study team for psychosocial and educational issues relating to patients’ acting disorders.
- Delivery of Support Services, individual, couple, and family counseling.
- Organize and facilitate support and education groups.
- Participate in weekly interdisciplinary clinics and team meetings.
- Provide expertise on psychosocial aspects of coagulation disorders to the hemophilia team.
- Coordinating community resources is necessary for maintaining the continuity of quality medical care by building and maintaining relationships with internal/external resources, and making referrals to community agencies and entitlement programs.
- Understand of all stages of growth and development to assess each patient’s age and specific needs to provide individual treatment and care.
- Evaluates patient s medical, psychosocial and social condition with respect to their current needs to establish appropriate post-hospital care plans and any services that may be necessary while hospitalized for patients diagnosed with inherited bleeding and clotting disorders
- Follows the documentation policy of the social work department
