Responsibilities:

Join our supportive and dedicated Older Person’s Community Team as a Social Worker, where you will be primarily working with service users within the Older People Community Social Work Team to assess the care needs of service users aged 65 and over.
The Adult Social Work Service undertake Assessments to identify eligible need(s) and formulate individual care plans with service users, families and carers. We promote independence, autonomy and dignity and draw upon strength-based approaches. We aim to maintain and enhance quality of life and wellbeing of older people by exploring family and friend support systems and local community-based support systems to enable older people to remain at home.
We are committed to maximising and empowering service users to make informed choices about their care needs. In addition to direct practice, you will also be involved in service development, consultation and other social work responsibilities.
