As part of Rennie Grove Peace Hospice Care multi-disciplinary team ensure patients / carers and their families’ holistic needs are assessed and a clear care plan is in place. To proactively contribute to the assessment and documentation of patient / carer and family needs within the Inpatient Unit, focusing on the psychosocial, welfare rights and practical aspects of their care. To report to the Head of Outpatients and Rehabilitation, with regular 1-1s, ensuring any service development adds value and meaning to the Outpatient Service and wider Patient Services and Rennie Grove Peace care strategic objectives and vision. Line manage Assistant Social Worker, with regular 1-1s and yearly appraisals completed.
To attend regular Protected Learning sessions and facilitate sessions to widen the clinical skills of patient services staff. To maintain own social worker peer support network across providers. Undertake service audits as required including the Safeguarding and Carers audit and encourage research / individua; service evaluation, collating quantitative and qualitative data of the service. To ensure operational policies, guidelines and risk assessments are in place to ensure the new service developments are in line with the wider hospice governance processes
