Join our DaVita team as a Social Worker (LCSW)! We are looking to hire a Part-Time resource who is able to support our patient population needs on-site at our Black Rock Dialysis Center (in Fairfield, CT)!
This hire would work a 24-Hour week, the schedule is flexible however this hire responsible for patient entire census. Once a month is required and must adjust schedule accordingly to attend Facility Health Meeting.

REQUIREMENTS:

• Master’s degree in Social Work (MSW) required with a specialization in clinical practice
• LCSW highly preferred, open to LMSW candidates as long as they are close and willing to sit for LCSW in the near future
• Meets all state required regulations to practice in the dialysis setting
• Demonstrated knowledge of government and private insurance programs
• Basic computer skills in MS Word, Excel, PowerPoint and Outlook as well as functional proficiency with DaVita specific applications
