About the roleDeliver high level clinical Social Work services as a member of the community based multidisciplinary allied health team, for adults/older persons within the Townsville University Hospital catchment.
The role works collaboratively within the multidisciplinary team, as well as across facilities and services to ensure the provision of a quality service, with a focus on complex health conditions, post-acute and rehabilitation focused interventions, and hospital avoidance and reablement programs.

Skills and experience

  • High level of knowledge, expertise and skill in providing clinical social work services with community-based clients presenting with chronic and aging health conditions and/or rehabilitation focused needs.
  • High level ability to effectively communicate within a multidisciplinary team and provide specific clinical advice to professional and operational supervisors regarding service delivery and improvement opportunities.
  • Strong ability to apply person centred practice principles to interactions and interventions with clients, peers, managers and community stakeholders, with a demonstrated understanding and valuing of individualised goal planning and self-management approaches.
  • High level ability to adapt to changing work requirements, applying strong flexibility, problem solving, organisational and time management skills.
  • Experience in providing supervision and support to clinicians, clinical assistants and/or students.
  • Commitment to ongoing learning and ability to contribute to continuous quality improvement.

Role requirements

  • Tertiary degree (or equivalent) qualification in Social Work and eligibility for membership with the Australian Association of Social Workers (AASW).
  • Drivers Licence: This position requires the incumbent to operate a class C motor vehicle and an appropriate licence endorsement to operate this vehicle is required. Proof of this endorsement must be provided prior to the commencement of duty.
  • Aged Care Pre-Employment Screening: Applicants are advised this position works in an identified Aged Care Facility as per the Aged Care Act 1997 and therefore requires employees and volunteers to have a current National Police Certificate. Queensland Health will facilitate applicants obtaining the above National Police Certificate.
