Description:

The incumbent will be responsible for providing comprehensive support across various organisation programmes, including Community Befriending (ElderAid), Meals with
Love, Young Hearts (YH), Medical Chaperone and Transportation (MCT) and Home Monitoring
and Eldercare (HoME+).

JOB DESCRIPTIONS:

Primary Responsibilities

  • Assess the suitability of referral cases to the programmes.
  • Facilitate referrals of clients to various assistance schemes and community programmes.
  • Provide advisory support for the programmes operation direction.
  • Establish and build professional relationships with client and next of kin such as family and care giver.
  • Identify, clarify and conduct needs assessment to assess the severity, issues, risks and protective factors for intervention.
  • To conduct phone interview and home visits in order to engage and monitor the clients.
  • Assess and identify the needs of programme beneficiaries in order to develop individualized support plans.
  • Develop and provide recommendations for consideration, monitor and evaluate the

progress achieving our goal.

  • Partner with agencies to ensure smooth transition of cases.

Stakeholder Management

  • Engage with community stakeholders to promote programme visibility and support.
  • Develop and maintain relationships with external partners to facilitate referrals and support services.

Administration

  • Maintain accurate and up-to-date records of beneficiary interactions and

programme activities.

  • Prepare regular reports on programme outcomes and impact for internal and external stakeholders.
  • Ensure compliance with organizational policies and relevant regulatory requirement.

Any Other Duties

  • Collaborate with other team members and volunteers to ensure effective service delivery.
  • Participate in professional development opportunities to enhance skills and knowledge relevant to the role.

JOB REQUIREMENTS:

  • Bachelor’s degree in Social Work or a related field
  • Registered Social Worker with the Singapore Association of Social Workers (SASW) is preferred.
  • Minimum of 2 years of experience in social work or related field, preferably with experience in eldercare, family services, or youth development.
  • Strong interpersonal and communication skills, with the ability to engage effectively with diverse populations.
  • Excellent organizational and time management skills, with the ability to handle

multiple tasks and meet deadlines.

  • Proficiency in Microsoft Office and experience with case management software.
  • Knowledge of relevant community resources and social services in Singapore.
  • Empathy, patience, and a genuine commitment to improving the lives of vulnerable

individuals and families.

  • Good presentation and report-writing skills
  • Good interpersonal skills, with the ability to relate well to people of all levels
  • Resourceful, innovative, good team player and able to work independently
  • Ability to empathise yet remain impartial
  • Enjoys working with volunteers
