Responsibilities:
ABOUT THE ROLE
As a Social Worker in our Transfer of Care/Hospital Discharge Team, you will play a critical role in ensuring that adults in Southwark receive the care they need to safely transition from hospital to home or other care settings. Your responsibilities will include:
- Conducting thorough, person-centred assessments to support safe and efficient hospital discharges.
- Collaborating with multi-disciplinary teams to create and implement care packages, including nursing and residential placements.
- Performing Mental Capacity Assessments and leading Best Interest Meetings to ensure the rights and needs of service users are prioritised.
- Handling safeguarding referrals and conducting inquiries to protect vulnerable individuals from harm.
- Staying current with legislation and best practices to ensure that our services are always at the cutting edge of social work.
