Responsibilities:
We are looking for a Social Worker that will have effective communication skills, be able to support and provide plans to patients and their families regarding social and emotional problems. They must show compassion while providing them with information regarding supportive and financial services that are related to their current hospitalization, illness, disability, and trauma. It is essential that this candidate has compassion for the patient and their families ongoing concerns and are helping to collaborate with them, while providing case management and milieu management.
The Social worker will help to complete psychosocial assessments, assist in the development and re-evaluation of Treatment Plans and work as a member of a multidisciplinary team, and to coordinate discharge planning. They work with a collaborative social services team and help to provide a supportive and compassionate environment for our patients.
We are looking for a #socialworker or #licensedsocialworker to join our #casemanagement team. The #socialworker will be handling#utilizationreview and the #socialservices for our#acutecare #hospital. The #socialwork done by our #LCSW and #MSW is so impartant to our #healthcare #enviroment. #LI-AM5
Qualifications:
Education and Work Experience
Required qualifications:
1. Master’s Degree in Social Work related field OR
2. Master’s Degree in Marriage and Family Therapy or related field.
3. In states without required licensure, then current registration with State Board is required within 3 months of hire. In states with required licensure, license required at time of hire and must be maintained.
4. Minimum one (1) year hospital or Outpatient experience with emphasis in mental health care.
5. Experience in working with a diverse population with a wide range of problems (i.e., death and dying, maternal and child care, substance abuse, disability, chronic illness, etc.).
Preferred qualifications:
1. Current BCLS certificate upon hire and maintain current preferred.
