Social Worker - Young Carers

at  TORFAEN COUNTY BOROUGH COUNCIL

AS A YOUNG CARERS SOCIAL WORKER YOU WILL:

• Contribute to the service direction of the Torfaen Carers, involving carers and relevant agencies, partners and organisations.
• Undertake Carers Assessments and Care and Support Plans to identify and respond to the needs of carers.
• Promote general awareness of carers issues amongst a wide range of organisations across health and social care and to the general public, encourage and support multi-agency working, and identify and contribute to training on carer awareness for professionals.
• Develop and support a network of local carers’ groups to provide platforms for carers’ views and identify gaps in service provision.
• Support nd supervise the Young Carers Community Support Worker and associated casual workers.
• Undertake other such related duties as may be required from the Director of Children & Family Services.
