Description:

EDUCATION AND EXPERIENCE REQUIREMENTS:

  • Master’s Degree in Social Work from an accredited university.
  • Two (2) years of recent, related experience working with youth and young adults with mental health and substance use challenges in a community health setting, or an equivalent combination of education, training, and experience.
  • Current full registration with the British Columbia College of Social Workers.

SKILLS AND ABILITIES

  • Knowledge of Addiction and Mental Health: Demonstrated understanding of addiction issues, mental health challenges, and evidence-based treatment practices.
  • Assessment Proficiency: Ability to conduct comprehensive psychosocial assessments using standardized tools for mental health and substance use.
  • Crisis Management: Skilled in managing complex clinical and psychosocial crises within the youth and young adult population.
  • Interpersonal Communication: Strong ability to establish rapport with diverse clients and communicate effectively with individuals and teams in a multicultural environment.
  • Organizational Skills: Capable of organizing, prioritizing workloads, and developing educational resources and support groups.
  • Knowledge of Relevant Legislation: Comprehensive understanding of relevant Acts, professional practice guidelines, and community resources for effective referrals and consultations.

Responsibilities:

  • Interprofessional Collaboration: Work as part of an interprofessional team to provide clinical services to individuals, primarily youth and young adults, affected by mental health and addiction issues.
  • Client Coordination: Coordinate the intake and transition of clients through the continuum of care. Assess clients, determine referral needs, and provide individual counseling, intensive case management, group therapy, and education.
  • Treatment Planning: Formulate and implement treatment plans. Provide comprehensive case consultation and follow-up support on treatment decisions in collaboration with interdisciplinary team members.
  • Team Participation: Participate in team and local committees, internal and external case conferences, and quality improvement activities.
  • Community Collaboration: Collaborate with other community service providers to deliver education and training related to mental health and addiction.
  • Program Development: Provide input into the development, evaluation, and delivery of the Inner City Youth Program
