QUALIFICATIONS:

  • Current California RN license
  • Excellent written and verbal communication skills
  • Car insurance, ability to travel throughout Orange County

Responsibilities:

  • Assess and admit patients for hospice care
  • Plan, coordinate, and implement health services for new patients
  • Provide clinical oversight of medications and plan of care
  • Review clinical documentation for consistency and accuracy
  • Deliver Explanation of Benefits (EOB’s) to patients
  • Advanced Care Planning
  • Provide director support and general clerical duties related to position
