Haven Health, the leading Home Health, Hospice, and Palliative Care Company in Southern California, is currently hiring an Admissions RN in the Orange County and Los Angeles area to fill our position within our Hospice division.

QUALIFICATIONS:

• Current California RN license
• Excellent written and verbal communication skills
• Car insurance, ability to travel throughout Orange County

• Assess and admit patients for hospice care
• Plan, coordinate, and implement health services for new patients
• Provide clinical oversight of medications and plan of care
• Review clinical documentation for consistency and accuracy
• Deliver Explanation of Benefits (EOB’s) to patients
• Advanced Care Planning
• Provide director support and general clerical duties related to position
