QUALIFICATIONS:
- Current California RN license
- Excellent written and verbal communication skills
- Car insurance, ability to travel throughout Orange County

Responsibilities:
- Assess and admit patients for hospice care
- Plan, coordinate, and implement health services for new patients
- Provide clinical oversight of medications and plan of care
- Review clinical documentation for consistency and accuracy
- Deliver Explanation of Benefits (EOB’s) to patients
- Advanced Care Planning
- Provide director support and general clerical duties related to position
