Description:
The purpose of this post is to support the delivery of transformation programmes across
Children and Young People’s services in Sutton. The overarching focus is to improve outcomes, access and lived experiences across health and care for children and young people and their families/carers in Sutton.
The post holder will support transformation programmes across Sutton Place (ICB and Sutton Council) and support the development of integrated working and delivery across Sutton’s system partners, providers and children and young people.
The post holder will be a member of the Sutton Alliance, Sutton Place and contribute to the Place team’s integrated objectives and deliverables and will ensure that their workstream of programmes are planned, managed effectively and will take the lead in assisting in their successful delivery.
The post holder would be expected to provide and receive highly complex, sensitive or contentious information, where agreement or cooperation is required. They will need strong interpersonal and relationship building skills with local providers, health and care professionals, politicians, local communities and citizens, and key stakeholders to gain credibility and trust. They will also be required to present complex, sensitive or contentious information to large groups.

NHS South West London Integrated Care Board (ICB), as part of South West London Integrated Care System (ICS), is a partnership of organisations that come together to plan and deliver joined up health and care services to improve the lives of people in our six boroughs: Croydon, Merton, Kingston, Richmond, Sutton and Wandsworth.
EACH ICS CONSISTS OF TWO STATUTORY ELEMENTS:
an Integrated Care Board, bringing the NHS together with its partners locally to improve health and care services
an Integrated Care Partnership (ICP): the broad alliance of organisations and representatives concerned with improving the care, health and wellbeing of the population, jointly convened by the ICB and local authorities in the area.
ICBs are statutory NHS bodies responsible for planning and allocating resources to meet the four core purposes of integrated care systems (ICSs):
to improve outcomes in population health and healthcare;
tackle inequalities in outcomes, experience and access;
enhance productivity and value for money and;
help the NHS support broader social and economic development.
NHS South West London Integrated Care Board decides how the South West London NHS budget is spent and develops plans to improve people’s health, deliver higher quality care, and better value for money.

Actively engage and work with colleagues and partners across the children’s and young people’s system, health and care and council staff and politicians to ensure transformation projects and commissioning programmes are clearly scoped and support high quality project delivery.
Develop project plans and manage and track delivery of allocated projects to ensure milestones are achieved on time, and proactively escalate any risks and issues to relevant project lead. Ensure risks are identified, managed, recorded and regularly reviewed.
Prepare and update reports for relevant programme, project groups and boards, and report regularly on project and progress and outcomes.
Work closely with enabling Strategic leads and the Head of Service to ensure adequate support is in place to transformation and commissioning programmes, for the Transformation and Commissioning Board, programmes, Health and Care plan coordination and updates, Service risk management and assurance meetings.
Collate, analyse and present complex data to stakeholders, adjusting communication style to the audience to ensure understanding.
Support the Head of Service and Strategic Leads in ensuring that contracts are compliant and performing in line with performance indicators specified, and remain strategically relevant in line with National, South West London and local strategic priorities.
Undertake regular audits to assess delivery of key performance indicators, before and during the transformation and/or commissioning project implementation.
Support the delivery of a strong performance management culture that supports continuous improvement, professional curiosity to demonstrate best value and best practice.
Ensure adherence to local and national policies, identifying any changes which impact on the project and making recommendations to incorporate changes into the project.
Develop protocols and procedures as part of the implementation and support to transformation and commissioning project delivery.
Work in an integrated manner with stakeholders and establish systemwide networks.
Effectively communicate with a wide range of stakeholders to build strong engagement within the projects
