The Specialist Children’s Community Nurse- Long Term Conditions will work within the Children’s Community Nursing Team in the community settings across Bedfordshire and Luton.
You will lead, develop and implement pathways for the nursing care of babies, children and young people (BCYP) with long term conditions across the Children’s Community and Specialist Nursing service.Supporting teams embed the relevant pathways into their clinical practice, and to provide education and training using your expertise.

It will essential to develop positive working relationships within an integrated community network to influence patient and professional experiences to ensure best outcomes. You will work together with the Specialist CCN-Palliative care Nurse and Continuing Care Teams to empower and support extraordinary babies, children, young people and their families living with long term conditions live ordinary live
Be an expert resource for Children’s Clinical/ skills and knowledge, providing education and training to the CCNSN Team and the wider multi-disciplinary team.
You may be required to participate in the on-call rota for the service.
Be the lead Clinician within the CCNSN Team supporting BCYP living with Long Term conditions ; including networking with specialist nurses to develop practice and sharing knowledge and skills with colleagues.
Lead, develop and establish integrated clinical care pathways across CCNSN services to ensure a consistent approach to case management/care coordination.
Responsible for day-to-day supervision of all staff as a senior nurse across the services. Act as a supervisor for staff managing children and young people with Long Term conditions.
Responsible for day-to-day management of a group of staff
Responsible for the development of staff across the countywide CCNSN service caring for children with Long Term conditions.

Main Duties
You will be expected to practice as a prescriber within an agreed scope of practice as part of this role.
Treat symptoms through pharmacological and non-pharmacological strategies. Undertake diagnostic testing within competence, e.g. Phlebotomy.
Empower health and social care professionals to deliver quality evidenced based care service by sharing knowledge and expertise.
To work therapeutically with children, young people, their families and other professionals
To develop and sustain effective working partnerships with local Clinicians, other specialist care providers, managers and team colleagues.
Understand and ensure good practice in managing patient information between the different care settings and teams.
Understanding of IT with an ability to use a range of IT systems that aim to improve communication within the services, including electronic patient records.
Budgetary Responsibility
To work with the Team Manager to ensure service remains within budget.
People Management
Responsible for day-to-day supervision of all staff as a senior nurse across the services. Act as a supervisor for staff managing children and young people with complex clinical care needs

Responsible for the development of staff across the countywide CCNSN service caring for children with complex clinical care.
Maintaining own professional development and requirement to take part in appraisal and KSF process.
Clinical and Practice Governance
Observe and maintain strict confidentiality with regards to any patient/family/staff/records and information in line with the requirements of the Data Protection Act.
Any data that is taken/shared as part of a phone call or transported, faxed, or transferred electronically must be undertaken with regard to the Trust Information Governance and Information Security policies.
The post holder must adhere to the Trust risk assessment and risk management processes.
The post holder must adhere to infection control policies and procedures.
It is a condition of your employment that you are currently registered with your professional body, and it is your responsibility to maintain your professional registration.
Undertake mandatory training and any other training relevant to the role as required by Cambridgeshire Community Services NHS Trust.
The post holder must participate in clinical and safeguarding audits as required.
Provide patients and their families /carers with information on standards they should expect from the team.
Participate in Clinical & Safeguarding supervision on a regular basis.
The post holder is required to participate in relevant emergency preparedness process for their team
