Description:
This is an exciting opportunity to join the cardiac division pharmacy team to support a new specialist lipid optimisation project being set up by MFT. A new clinic aims to review all post MI/ACS patients to ensure their lipid therapy is being optimised. The post holder will provide an independent review of lipid therapy and provide recommendations to changes in therapy and help to reduce health inequalities across this patient population.
Post MI patients are currently discharged back to their GP for follow up, and data shows a lipid review is only occuring in around a level of 30-40% of patients. As such, this equates to approximately 60-70% of patients from Greater Manchester not reaching their LDL-C target of This project, supported by GMMMG and Health Innovation Manchester, aims to evaluate the impact a pharmacist led lipid optimisation clinic has on post ACS/MI patients lipid targets. The clinic will work to ensure all patients have lipids optimised in accordance with the GMMG lipid pathway and Nice guidance. Project data will be used to develop a proposal for consideration by the Greater Manchester ICB, to gain on-going funding for this service.
The post holder will primarily be responsible for the set up and implement a new lipid optimisation clinic in conjunction with the lead consultant and pharmacy cardiology directorate lead as part of the Cardiac Division pharmacy workstreams.
The post holder will also support the wider pharmacy cardiac team by providing a high-quality pharmacy service to allocated clinical areas, ensuring timely and accurate medicines reconciliation on admission, ongoing inpatient monitoring and review in accordance with agreed acuity and complexity requirements, and optimising medications to deliver a safe and appropriate discharge service to patients. They will support the directorate lead pharmacist and be expected to actively participate in organisational medicines safety and risk management initiatives, including audit and incident reporting.
