Description:

We have an exciting new opportunity for a part-time Band 7 Dietitian (12 months fixed term with possible extension) to join our community team to lead on the implementation of the locally developed malnutrition management pathway focusing on providing training, education and advocacy. The role also involves developing working relationships with Primary Care Networks to influence the implementation of best practice for the management of malnutrition and hydration.
The community dietetic service includes a successful and proactive team of dietitians and dietetic assistant practitioners covering the three inner boroughs of London (Westminster, Kensington & Chelsea, and Hammersmith & Fulham).

We meet face-to-face once a month and virtually once a week to support communication, team building and staff well-being. The team has access to office space across the boroughs as well as the option of some working from home.

The post holder will be part of a wider network of Dietitians throughout the Trust. We encourage and expect all staff to participate in Quality Improvement. There is a strong emphasis on learning and development and delivering personalised care and a health coaching approach to those accessing our service. Training and supervision for Health Coaching will be provided for this.

  • Lead on the tri-borough implementation and monitoring of the trust wide malnutrition management pathway for community (predominately nursing) teams within CLCH including the local directory resource for signposting.
  • Work with the appropriate prescribing lead on developing and implementing a training programme that supports Primary Care Networks to identify and manage malnutrition, support good hydration as well as an improved understanding of the role of prescribed oral nutritional supplements.
  • Lead on engaging and developing working relationships with Primary Care Networks to influence the implementation of best practice for the management of malnutrition and hydration
  • Be able to identify and advise on highly skilled integrated and holistic care as part of a population health management approach or for individuals during multi-disciplinary team meetings.
  • Lead on multidisciplinary and multiagency working (protocol and policy development) in areas of clinical speciality to inform service development in the specialist area of nutrition support.
