Specialist Heart Failure Nurse

The successful candidate will join the established heart failure team, supporting patients within the borough diagnosed with heart failure. They will optimise the care and management of these patients in line with local and national guidelines and in collaboration with primary and secondary care providers to ensure seamless care and management of the complex field of cardiology.
The Heart Failure Specialist Nurse will primarily be responsible for managing a caseload of patients with Heart Failure in the community.
To Support this, the Heart Failure Specialist Nurse will liaise with other members of the heart failure management team across primary and secondary care to deliver evidence based care to patients with Heart Failure.
The Heart Failure Specialist nurse will also co-ordinate an education programme for community health professionals and act as a resource for advice.
To assess and treat patients within the speciality who have highly complex physical, psychological and social needs and work as an autonomous practitioner in delivering heart failure care
